1. The Course Agreement is a very short, 1-page form that limits our liability, confirms that you know that this is not psychotherapy nor a substitute for psychotherapy, asks that you agree not to take any non-prescription drugs or alcohol within 24 hours of each session, and gives the exact course dates and times. Click one of the link choices below, depending on which format you prefer. It asks your name and address, sign it, print/mail/fax it, and you’re done (parents, please sign for kids under 18).

2. The Course Information Sheet is an in-depth, 4-page questionnaire that helps you get clear on your family of origin and your unique past. It gives the instructors more info about who you are, where you came from and what you want out of the class, so we can best help you get there. People report that their class really begins when they complete this form. Click below, complete the form as honestly and completely as you can, and print/mail/fax it in (totally confidential)… details are provided on the form itself.
